Permanent Supportive Housing offers homeless persons with disabilities safe, affordable places to live along with support and resources. PSH enables residents to improve their well-being and living conditions while becoming self-sufficient citizens.
These options are for people who also have health concerns such as mental illness, chemical dependency, HIV/AIDS, and/or other disabilities. Applicants must meet the HUD definition of homelessness, and be at least 18 years of age. Participants are expected to engage in services and maintain a sober lifestyle. Permanent Supportive Housing is available at scattered sites and individuals apartments throughout Woonsocket and northern RI.

Support Services Available
- case management
- life skills training
- relapse prevention
- support groups
- employment readiness
- GED instruction and other educational support
- Must meet the HUD definition of homeless/low income and standard for disability
- Be 18 years of age or older
- Participation in outpatient mental health treatment, and substance use disorder treatment as indicated by diagnosis
- Participation in a support group is recommended
- Be able to co-exist peacefully and abide by the terms of the apartment lease
- Other eligibility requirements as determined by each housing option
The Women’s Program
Constitution Hill Supportive Housing Program - A self-governed sober living community at the single-family Women’s House, with five private bedrooms. Must meet definition of chronic homelessness.

The Men’s Program
Burnside Permanent Supportive Housing Program - Burnside offers single men housing in an apartment building with nine one-bedroom units. Each has a kitchen, living area and bedroom. A smoke and substance-free environment. Must meet definition of chronic homelessness.

Housing Opportunities for Persons With AIDS (HOPWA)
The Agape Permanent Supportive Housing program offers housing opportunities to members of the community who are homeless, low-income, and living with HIV/AIDS. Single applicants as well as families are encouraged to apply. Apartments are either 1 or 2 bedroom units located at sites throughout Woonsocket.
