Walk Ins Are Welcome at 800 Clinton Street!

Your Intake Assessment will be done in order of arrival. When you arrive, we will inform you of the expected wait time.  Accepted Insurances: Managed Medicaid, Commercial United. Individuals with Medicare, BC/BS or Tri-Care should call 235-7121 to schedule an appointment in advance.

All walk-in intakes are done at 800 Clinton Street, Woonsocket.

There are times when special circumstances, parental permission, or additional information is required before your child can be seen.

If you need to schedule an appointment due to work or transportation concerns, contact intake 401-235-7121. If you are 15 minutes late for your scheduled appointment, you will have to reschedule.

Please Bring with You

Remember to bring your insurance card and a picture ID. We will help you apply for financial assistance to pay for your services if you are eligible. In order to complete the application, one document of each of the following are required: 

  • Identification
  • Proof of Rhode Island Residency
  • Proof of Citizenship or Lawful Permanent Residency (five years)
  • Proof of Income
  • Proof of Insurance Coverage
