First Connections

Our First Connections program provides free, voluntary, confidential home visits to women who are pregnant and families with young children from birth to age three.

We provide visits in Burrillville, Cumberland, Foster, Glocester, Johnston, Lincoln, North Providence, North Smithfield, Scituate, Smithfield and Woonsocket.  Anyone may offer to arrange a home visit for a family.

What we do:

Engage families

Conduct home visits

We meet with families in their homes to talk about their needs, answer questions, and conduct comprehensive assessments. If needed, arrangements for interpretation services during home visits can be arranged.

Link Families with Services

Based on the needs of the family, we provide  health education and connections with appropriate healthcare services, human services and community resources. We also conduct child wellness screenings and communicate the results with the child’s healthcare provider.

First Connections is funded by the Rhode Island Department of Health and Department of Human Services.
