What Does a Community Health Worker Do?
Our certified Community Health Workers (CHW) can provide:
- Health education and support
- Health promotion and coaching
- Assistance navigating healthcare systems
- Support accessing appropriate health and community resources
- Assistance understanding and following healthcare recommendations
- Support with connecting to services that may help address health-related needs
Community Health Workers do not provide medical diagnosis, clinical treatment, or psychotherapy.
Who May Benefit from CHW Services?
CHW services are intended to support individuals with identified health needs and may be appropriate for individuals with chronic physical or behavioral health conditions or documented health risks.
For individuals receiving Rhode Island Medicaid-covered CHW services, additional eligibility, medical necessity, authorization, and program requirements apply. Health-related social needs such as transportation, housing, or food insecurity may be considered as part of an individual’s circumstances but, by themselves, do not establish medical necessity for Medicaid CHW services.
