Healthy Transitions

|
Our team works together to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood.
|
For youth and young adults with serious mental health concerns the transition from adolescence to adulthood can be challenging, especially when facing new roles, responsibilities and expectations for independence. Health Transitions provides a non-judgmental and encouraging environment in which to understand and work on symptom management at one's own pace.
Connected
Not only will participants be connected to their service providers, but they will also feel connected to other participants in the program.
Accepted
Participants make their own decisions without fear of judgment.

Participants create their own personalized treatment plan with the help of the team. They work with as many or as few members of the team as they choose.
